ADOLESCENT TRANSITION WORKSHOP

ConnecTeD Foundation’s adolescent transition program is designed to support adolescents and young adults between the ages of 13 – 19, in the move from child-centred health to adult oriented health care. Our program provides an overview of what to expect as a child with a chronic illness moving into adulthood and the changes that occur, including:

  • ·Finding your adult voice, your rights and responsibilities
  • ·The continued role of family and friends
  • ·Maintaining support networks
  • ·Emotional well-being and other body changes
  • ·Keeping and maintaining your own health file
  • ·Managing specialist appointments
  • ·Support groups and what they can offer

Our transition program will be run by:

  • ·Renee Veliscek-Plisek (ConnecTeD Foundation Board member, Allied Health Professional and person affected by CTDs)
  • ·Adam Heaney (ConnecTeD Foundation President, Allied Health Professional and father of person affected by CTDs)

We limit our sessions to 20 participants so be quick to join us for the Inaugural Marfans Syndrome & Related Disorders Conference in Melbourne 2015!
