Planning supported transitions for young people with intellectual disability: paediatrics to adult healthcare services

The session will discuss transition planning for adolescents with intellectual disability proceeding into adult lifestyles and services, including case study for reflection and resources for local implementation and use.

Learning objectives include:

1. Expanding your understanding of intellectual disability

2. Understanding the role of GPs within the transition process from paediatric to adult services within healthcare

3. Increase knowledge about restrictive practices and medications

4. Learn about services and resources available to support the transition process
