MPHN Care Finder Consumer Consultation - Berrigan

MPHN will be conducting a series of face to face consumer and community consultations to help develop a new Care Finder Program which will begin operating in January 2023 across the region.

This program is being developed and implemented by all Primary Health Networks to support senior Australians and their families and carers to access aged care services and connect with other relevant health and social supports.

To support the development of this program, we would like to talk to older people in your local region regarding their experiences in accessing local aged care services and how the Care Finder program could ensure best access to these services.
